USTAR Online Visual Tour (Registration Step 2)
The Registration form needs to be completed and submitted before you can use the USTAR System.
Enter the 14-digit Account Number and the Administrative Password printed on the Quarterly Contribution and Wage Report mailed to you.

A valid email address is required to continue your registration.
Click 'Submit' to enter your information in the USTAR system. You will receive
an email containing a temporary password to login to the system.
