Power of Attorney

A business/taxpayer may appoint a representative to act on its behalf in unemployment insurance tax and/or claims matters with the Division of Employment Security, with the exception of the Division's appeals process.

A signed, written agreement must be submitted to the Division.  The Division provides a Power of Attorney form that may be used.  The completed document should be mailed or faxed to:
