If your company has authority to write worker's compensation insurance in Missouri your company is required to report even if no Missouri premiums are written.
Your surcharge form and check must be received by the 30th day of the month following the end of the quarter as indicated on the quarterly form.
There is a form for each quarter. The date that forms checks must be received is indicated on the top right hand corner of the quarterly form.
Mail your company's form(s) and check(s) to the Division of Workers' Compensation to the address on the form.
One form is required for each company for which surcharge is being submitted. You may submit one check for all companies but the Division must have a form completed that indicates the premium written for each company.
